It would be considered forgery if the girlfriend signed your husband's name to a legal document. However, you'd have to be able to prove it and file a police report regarding the incident. If his name was forged, then I believe it would nullify the document. However, you may want to ask whether you're not better off single than with an unafithful man headed to prison. (Incidentally, typing in all caps is the cyber-equivalent of shouting.)

Are you kidding us? Girl, look at the situation! You have a man to whom you’re legally married who's been unfaithful, has done something that warrants prison time and you wonder about staying attached to him? This woman whom ever she is has done you a huge, great favor! Her signature or his sign at once then send her a thank you card! No one in you're situation is in a good place because your self esteem is in the toilet! Prison is a shame on the human condition. Yes, some are not guilty but 99% are and in there for shameful things! Here is your chance to look where you are and make a new start. Take it because opportunity only knocks very few times. This guy is dirt!

Start all over again with the papers. you are going to have to observe your jailbird husband sign these documents in person. Girlfriends don't count.

check with a lawyer in your state. Even if the documet is forged, you can't "choose" it to become "void" (by which I assume you mean of no force and effect). If it has been filed and served-- properly-- you must follow proper procedures. Expensive procedures. Why do you care who signed it? Does it have inaccuracies? Can you stay married, if it turns out the Petition or complaint is forged? Would you want to? Other than drama, what is the problem here?
