If you quit a job in NY due to health reasons, that the job itself is causing mental stress, anxiety and panic attacks. Are you able to collect unemployment benefits? Would a doctors note be make it definite?

No. You cannot collect unemployment if you are "disabled? The whole point of unemployment is to assist those, who under circumstances were let go (fired) or excised from their job. Go down to unemployment services and ask them this question hypothetically to see what they say. Sometimes they can help you out! Peace and good luck!

I'd go for state disability instead since you quit with cause: illness. and Yes, definitely a doctor's note as this will probably up an uphill fight, but one worth fighting!

If your doctor is willing to testify that he ordered you to quit for your health, then maybe. You would probably have to got through a lot of hoops (denials, appeals, etc) before your were approved.
