There used to be a place in Indy where teachers could go (and so could the public) for "educational supplies". They had a number of gifts for teachers and students. They had some ceramic apples (kids used to give teachers real apples, you know)... Or something that will remind her of you, specifically. Do you have a nickname? Anything come to mind from that? Or, write her a poem... about her and the class. Print it, frame it, and wrap it for her. ;-)

Teachers ALWAYS appreciate a card. And like any other gift, one that is personalized, or made by the giver, carries more significance. Stay away from gifts that are obviously monitarily linked: cash, gift cards, and so forth. Teacher related items are welcome: a book on math puzzles, a personalized pen, or a gag gift related to math, in this case. A paperweight for the desk or some such knick knack. Getting the entire class together for a special class picture and then everybody signing it would be an AWESOME gift, especially if you guys frame it up in your own custom frame.
