It could be a lot of things, but is very unlikely to be a fungus. The easiest thing it could be is a cyst, in which case you should *stop* putting any cream on it at all: cream will just make the blockage worse. The worst it could be is a tumour, in which case the cream is beside the point. You need to take your dog to the vet and get this bump diagnosed, so you can treat it properly.

Shih-Tzus are prone to skin issues. I know. I've spent a small fortune on trying to keep GiGi well. The sore is likely something that will respond well to Neosporin. I use it on GiGi when she scratches a sore place and usually within a day or two, it is healing up. Open wounds are different than hot spots and may require more aggressive treatment from your vet.

It sounds like the same thing my dog had a couple years ago and OF COURSE I can't remember what its called. Bottom line - you need to go to the vet. If it IS the same thing the sore will grow very quickly!! The treatment is very simple - Vet gave us some type of medicated powder that had to be applied several times a day. Good luck!
