Next of kin or whoever is directed to receive it which would be addressed before the ceremony. They would usually take the initiative to ask who will be receiving the flag. I was in the Air Force Honor Guard and we had ample notification time for last minute changes. Also everything is coordinated days before the funeral. Hope this helped...

Also, if there is no next-of kin or relative available, an individual can step up and ask to be the one whom the flag is handed to. If this isn't able to occur, the military themselves will have friends of the individual (hopefully) who can step up for the flag and worst-case scenario, a representative of the service division steps in and takes the flag.
