The bacteria come from outside the body, up through your urethra, and then into the bladder. On a related note, the reasons why women get more bladder infections than men is because of the extra moisture, the bacteria don't have as far to go, and rubbing during sex can help move bacteria up and into the urethra area. So, gynecologists and urologists (at least the ones I've seen) advise women to urinate right before and after sex to help flush away the bacteria.
