I flag a "question" offensive, but the tag is "spam/offensive" and when I continue it says something like "Do you want to flag this as spam?" Does it get flagged as spam or spam/offensive? Will a mod know or care?

It doesn't matter, really. In either case, the questions will get removed from the board, if a staff mod agrees. Yes... They can tell the difference between spam and offensive. Spam almost always includes a URL to a site requiring signup, membership subscriptions, or a shopping site. Offensive is... obvious.
