For outright cruelty, I would have to say Vladislav III (the inspiration for Count Dracula, a title he held). He became known as "Vlad the Impaler" due to his favorite method of execution.

LOL @ all the people who put Hussein ahead of Hitler, Mussolini, Stalin, Mao, et al. I don't think any less of them, I'm just amused. Anyways, I'll go with Stalin. I'm surprised only one other person has put it, I guess people don't realize how horrible the purges were.
