It's just a matter of exposure.

Even if you suntan flat on your back with your arms extended outwards and the undersides facing the sun, the rest of your activities normally expose the upper sides of your arms –– when jogging, walking, driving, boating, fishing or whatever.

This can also be noted in people who freckle, they generally have more freckles on their face, upper sides of their arms, fronts of their legs, shoulders etc than the skin which is less exposed to the sun.

If you were to ensure even exposure to the two sides over a prolonged period, there is no reason why the underside should not catch up.
