You probably already checked but make sure the cable is securely attached to the back of your computer. Update your video card software. Most unlikely it could be a faulty monitor but I would say just off the top of my head it is a video card issue. Try connecting the monitor to another computer to verify. If it works fine on another computer it is the video card.

Sounds like a Monitor problem to me. Find out what "native" resolution the monitor has and set your video adaptor to output that resolution both in Windows and the various applications you use with Windows. That way the monitor no longer has to "auto config" every time you change applications. If you don't want to do this... go into the monitor set-up and disable input scanning and the message will only pop up when actually changing applications. Hope this helps.
