I don't know where the term comes from, but it is definitely not limited to the South. My business partner was scolded by a geology professor when he used the term in a field report back in the '60s. He is of Norwegian descent and at that time had lived in Thief River Falls, MN all his life.
It is a Southernism, which means very, very flat. And here's a source to document this, which also defines other southernisms such as "cotton pickin'" and "no bigger than a minute":

In Southern Indiana, Flitters are flat dumplings. They are made with flour, eggs & a little milk, and rolled out into a very flat dough. Then they are cut in diamonds or squares and left to dry. After they're dried, they are slowly cooked in a rich chicken broth. Best darn comfort food you ever tasted!
"Flat as a Flitter" simply means as flat as a noodle, because that's how flat flitters are.
