The 401k administrator. It is extremely rare that companies administer their own 401k plans - usually it's Fidelity, or Vanguard, or some other financial institution.

You might contact the tax assessors or revenue office. Occasionally when people are owed money and the agency or company is unable to locate them, the money is placed in a trust and in my state, you can look up on-line if you have 'unclaimed money'. This might include closed bank accounts, unrefunded deposits for rental property or utility service, or unrefunded 401K contributions. You might just try searching 'unclaimed funds' in your local state governments website. It might be easier than you think to recover your money.
