When they defecate on themselves it's not just that muscles that are normally tense relax, but the noxious "decay-gas" trapped inside the body put pressure on the remaining fecal/uric matter, and it is expelled. It's the same way that an uncooked sausage sometimes splits it's skin: things like botulism. Remember, not everyone who defecates on themselves in an accident is actually knocked out. Sometimes the reason an unconscious person has fecal matter on themselves is because they watched the thing that was going to knock them out hit them, and in sheer terror, they emptied their bowels before they were hit. But for the most general of cases, it's because the sphincter muscles are no longer tense.
