If your instinct to procreate is so great that you would go as far as to either do so with a child or a family member, then I see no reason why you would not be willing to do so with both. It's evolutionarily logical to want to reproduce, some just have it more strongly than others.

I consider it a matter of opportunity. If someone is wont to molest children, then the chances are good that they will molest whichever child they can most easily get their hands on, and if you're an adult in a child's family, then you'll probably be granted some power over them. In other words, the reason so much pedophelia is also incestuous is because it's so much easier to molest your own young family members than it is to go out and try to rape a stranger. And of course, I doubt many pedophiles have some strong ethical aversion to incest.
