Yes, I have an Advance Health Care Directive that explains what I will and won't accept in terms of medical treatment, and gives POA to my daughter should I be incapacitated. I've had this since my daughter became an adult. I think it's neecessary to have a document like this in place bcause you never can predict when something might happen to you.
