They can vary greatly. Even though any Spanish speaking person can make himself or herself understood anywhere Spanish is the official language spoken, there are geographical, ethnical and cultural differences. On Latin America, for example Argentinian accents and modisms are completely different to Mexican ones or the expressions and accents of a Spaniard, for that matter.

They vary greatly even within the tiny island I live on. I can understand some people easily, yet I have real difficulty in understanding people from the rural areas.

In Spain itself the dialects, like in Latin America can be very different. The language is Spanish but sometimes the dialect is so different it is difficult to understand. I believe this is very much the same in most countries . In England some people have accents that are very difficult to understand.

It's basically like the United States. In the South people have a different accent, they use different words (like y'all), in the Northeast, the Midwest... even Canada. It's all different dialects, but we all speak Enlgish.
