I would lean towards a terrorist organization getting a dirty bomb set off somewhere then a country launching a full nuke.

No way to tell. So many people can get ahold of nukes these days, that there is no way to tell who plans on using them on us. Best we can do is keep a watchful eye, and hope that someone in intel knows what they are doing.
