Based on what I have learned, the back-story of the Joker's skin being "permanently dyed" isn't in his earliest comic book appearances. He's just a mass murderer with an evil laugh and a clown-like appearance. The way he looks is never really explained. I understand that Ledger's Joker is more true to the character's original origin.

It's makeup... anyone remember the one scene where he's disguised and not wearing the makeup? You see a flash of his scarred up face with no makeup. I don't want to spoil it for anyone so i won't go into detail.
