The Joker, he's caused Batman the most suffering. He crippled Batgirl and "killed" the second Robin. Batman tried to kill him once, but was stopped by Commissioner Gordon.

I always took it to be the Joker, particularly when "The Killing Joke" was published. The Joker was sort of a shadowy reflection of what the Batman could have been, if he had reacted one way to a tragic set of circumstances rather than another...
