It is not so much whether they were of sound mind at the time of making or changing the will, as whether you would be able to prove it in court if the will were contested after their death. If you do not have an impartial and reasonably competent witness, it would be very difficult to contest the prima facie evidence from their medication that they were not of sound mind.

Sure but you might want to get confirmation (at the time the will is changed) from a professional(Dr. Physician,Psychiatrist etc). Also, this is one instance where you would want to videotape his/her reading/signing the will....just in case
