In the early days of the internet, the most common form of communication was through email or through the USENET (newsgroups). At this point, everything was on a terminal running on a mainframe type computer, and there were no graphical programs to include italics, bold, or other emphasis to text. People began assigning meaning to certain characters or customs. One could use asterisks as *emphasis*, for example. Given this, it is obvious how all caps came to be seen as shouting. It is effectively an extreme form of bolding in a text-only system. Research has also shown that uppercase is harder to read than lowercase, so the effect of using all caps is that it "hurts" your eyes a little to read it, just as shouting "hurts" your ears.
