It has to do with two primary things (same with movies). First: Who gets top billing based on salary which is based on negotiation. Second: Who has the most market appeal to lure advertisers to pay to have their names associated with the show.

It has a lot to do with money. When the actor is cast they might accept less cash if they get higher billing. Vanity has it's price. Sometimes it's just stupid. My favorite example of this was on the Patty Duke show when they had another cousin stop by for a visit... The credits read "Starring Patty Duke as Cathy, also Starring Patty Duke as Cathy.... Special Guest Star Patty Duke"
