The shelter where I adopted my dog said he was neutered, but he still has a scrotum and still humps once in a while. Does that mean he wasn't neutered?

The term "neuter" implies the testicles were removed. The scrotum is the skin holding the testicles. The testicles could be removed and artificial ones could be inserted. I doubt if your shelter would go to that extreme. To answer your question you need to determine if the testicles are inside that scrotum or not. If you can't tell by looking then you'd have to feel for them. -
