If they used it all the time, then I think it would, but since some of the away missions are done using shuttles, the possibility of contamination is not zero. I suppose that they could incorporate a transporter filter on the docking decks but where is the fun in that? Besides, someone has to heal the crew after they hurt themselves on the Holo-Deck.

No. The problem with transporters is that they have a maximum range of 40,000km. On long journeys, there may not be a hospital within that range. Thus, sick bay is always a necessity.

You're missing the point entirely! If the transporter can recreate you whole without any disorders or injuries why would you need a sick bay? If you were injured or developed any disorder just beam out and back again whole without the injured portion. Just use a recording you whole for the recreation!

No, because the transporter is not designed for medical use. Its purpose is to transport a person from place to place WITHOUT making any alterations.
