You need to replace the rubber gaskets,and if they cannot be found buy a new faucet.

Some handles have a front cap that has to be pried or popped off,others have a very,very small set screw that requires a miniature screwdriver to remove the set screw.

Sounds like a washer or seal has worn out. Get a plumber, its obvious you will cause a major flood disaster if you start taking it apart yourself!
