I know someone who is constantly complaining about feelings of inadequacy and insecurities, but never makes even the smallest step towards changing the causes of the problems. It's very frustrating. Should I just stay out of it or try to help?

I try to help people like that, but I'm not going to talk to a wall. You can only say and so so much before their self pity becomes an annoying problem to you. If he/she tries in earnest to help theirself, fine-- if not, just let him/her stew in his or her own self inflicted hardships.

I have had a friend like that for 15yrs I finally accepted the fact that they would never change. We are still friends and I do sometimes still try to help but I don't expect anything to change any more!!
