Depends on the type of blessing or the religion. In a lot of cases a blessing is performed just with a hand placed apon the head and none of the annointing accessories. I have seen a Baby Baptised with water straight from a tap in a hospital room and blessed.

They aren't mutually exclusive. In general, my blessings include all of the elements: Earth, Air, Fire, and Water. For simple blessings, saltwater (Water & Earth) and incense (Air & Fire) are used. I prefer smudging for banishing negativity; however, the herbs I chose for the smudging are dependent on the reason it is being done.

In the context of Catholic, I believe it differs.. water for general blessing and oil for extreme unction. Salt is not really acknowledged, nor is smudging (unless I misunderstand you). Incense is extremely blessing and cleansing, especially copal, myrrh, pinon and benzoin.
