  • Is your wife still alive? If so, then you must both be active faithful members for at least one year. This means that you must be obeying the laws of tithing, chastity, and the Word of Wisdom. You must also be fulfilling your callings. Specifically, this means that you should be doing your Home Teaching and you must hold the Melchizedek Priesthood. If you are doing these things, then you go to your Bishop or Branch President and ask for a temple recommend. He will interview each of you separately. Once he has determined that you are worthy, he will fill out and sign a recommend for each of your. (As you are asking this question, I am going to assume that you have never been to the temple before. So, what you will getting is a recommend for your own ordinances.) Once he has signed these recommends, you will need to take them to your Stake or District President. He will interview you again and then, if you are worthy, also sign the recommends. Once you have your completed recommends, you then take them to the temple of your choice at a time when they are performing the ordinances for the living. You will also need to obtain a set of temple cloths. (Your Relief Society President will be able to tell you where you can purchase your own or you can rent the cloths at the temple for just the cost of laundering them after you are done (it was less then $3 the last time I did this).) You will also need a set of garments that you will keep. In the temple you will first go through the Endowment Ordinance. After this, you will move to one of the Sealing Rooms to be sealed as husband and wife for eternity. If you have children, then they too should do that which is necessary to go with you so that they too can be sealed to you, though they don't have to in order for you and your wife to be sealed. Additionally, I would recommend taking a Temple Prep class at church. In this class you will be told in general terms just what to expect as you go through the ordinances. If your ward/branch is not offering the class then some other endowed member can tell you what you need to know. Additionally, it is traditional to have an endowed family member or close friend serve as an escort when go to the temple for your own ordinances (a male for you and a female for your wife). In my case it was my parents that told me what to expect and my father who served as my escort. That same day that I received my endowment my parents and I also performed the ordinances to seal my mother's parents and her older brother to each other. (I stood in for my dead uncle.) Now, if your wife is dead, the answer remains mostly the same. However, your wife will have to have been dead for at least a year and you will have to find an endowed female member to perform the ordinances on her behalf. If there is no one in your ward or branch who you are comfortable asking to do this, then one of the people who works at the temple would probably be willing to stand in for her during the sealing ordinance and you could arrange for her name to be give to one of the sisters who is there that day to do the endowment for one who is dead. This is a general outline of what you will need to do. You should consult with your local leader on this though. They can be a lot more specific than I feel like I can be in a open forum such as this.
