If you want to leave the Church, you simply need to contact the Bishop of the ward in which you live and tell him that you want your name removed from the rolls of the Church. Miss Zee, you actually don't need to speak with the bishop or anyone else for that matter. Should you desire to have your name removed all that you need to do is to write him a letter requesting such. When I did so I also copied my Stake President and sent a copy to the Membership Records Department in Salt Lake. Your bishop and stake president should be enough though.
