Yes, some women can orgasm by fondling or playing with her clitoris without the aid of vibrators or sex toys. The clitoris does not have to be "swelled." Although I'm not entirely sure what you mean by "swelled."

Yes you can orgasm by just playing with your clitoris,
and the clit will become swelled from this. Some women have a hard time cuming with out toys and some have a hard time with them. Its truly and individual experience. and all women are different.

The swelling of the clitoris just means that your blood preasure is us and your juices are flowing. You can have an orgasm just by masturbating with your fingers. But, masturbating with your fingers will probably lead to a swelling of the clitoris before you have an orgasm.
