I don't think it has happened in the NBA but it has happened in college a few times. It is usually related to heart trouble. It is a condition that is more common in really tall people. Here's an article about a teenager that collapsed and died from a heart attack.

I can't remember his name at this time, but one man died while playing basketball, from a heart attack. He had an undiagnosed heart condition, and the stress of the game finally did him in.
As I recall, he was only 17.
