I always found it was well received to include a few kind (but honest!) words for the boss(es) or co-workers that helped me in my position(s). The kudos can be filed in their personnel files and thoughts, impacting their careers. These same people may even be valuable resources and references in your future. Beyond that, a simple communication of your intentions to leave the company and your last available day to work are sufficient, preferably giving two weeks notice or more.
