Answering for my wife. No, but her maiden name and mine start with the same letter. So, she did keep her initials. My mother uses her maiden name as her middle name, with no hyphen.

The first time I got married, I took my husband's name and kept it even after we divorced. The second time I married, I took my husband's name, but went back to my maiden name after we divorced.

No, I hypenated. Then we I got divorced I changed back to my maiden name. If I get married again, I will not change my name, it's too much of a pain in the butt.
