SSI (part of Social Security in general) and Medicare were devised with the notion that each generation there would be more young to support the old. The Post War Baby Boom was followed by a baby implosion - the boomers had less children, so now we have a population of more old folk than children to support them. Health care would be based on immediate population numbers who are paying into the system and taking out of the system. We would be hard pressed to end up with a majority of users and few payers. Unless the majority suddenly became ill to the point of being disabled (unable to pay).
