Baby oil and vaseline. I like to heat up the vaseline so it's warm and a little more liquefied. But the clean up is a pain. I like water soluble liquids, like KY or Astro Glide but my wife prefers our SUPER lube, especially for anal.

KY almost exclusively. We have tried lots of other stuff but we always come back to KY. Sometimes in the shower I fuck her ass with baby oil, but KY is the standard and safest and generally just works without worrying about cleanup, etc.
