I'll generally do something embarrassing like pick my nose, say something inappropriate or something specific to the environment.

Probably just stare back until they look away, or ignore them.

I got a real shock at the Dr's office awhile back. They had a TV mounted above my head in the waiting room, and a mirror across the room. Since the place was faily full, I couldn't move to see the show, and besides, I could hear it quite well where I was. But after a few minutes, a woman suddenly started shouting at me: "STOP STARING AT ME!!!!" I quickly explained that I was just watching the show in the mirror above her. She seemed grudgingly understanding, but certainly not apologetic.
