I think that may have been done partly with illustrator.
You could do it in Pshop however...copy the main image as a layer several times then on one play with circle brush and its attributes and paint to delete the square edges; on second layer, underneath first, lighten opacity and delete pretty much the same leaving a little more, and do again on 3rd. Can use waterpaint type effects also.
