Yes -- you must fill in all the other line items. By filing the 1040X, you replace fully the 1040 that you filed. In the future, you (and the IRS) will refer to just one return for the calendar year in question, and it will be the 1040X.

Yes, you will fill in the column 'as originally reported' and then the last colum that is 'correct amount'. The $8000 credit will appear in the middle column under the correct credit and then the column to the right for 'correct amount'. And on page 2, you will put in 'correcting to claim homebuyer credit: Adding form 5405' under the explanation. You will need to send in the two pages to the 1040x, two pages of 1040, and the form 5405.
