I think it would be nice! I worked with a friend who was way more than well-endowed. She use to complain about her back all of the time and told us she was saving money to get a reduction. She took her vacation time and came back looking so much better. She looked happier, younger, and she kind of bounced around like she was lighter after she got her reduction. She loved when anyone mentioned it and would tell them that it was the best thing she's ever done for herself!

Kinda depends on her and how you bring up the subject. No pun intended here, but such a topic can be a 'touchy' thing with women. What some find complimentary, others may find rude or embarrassing. However, as a rule of thumb, if SHE brings up the topic, then YOU can generally make an appropriate comment in reply.

If this surgery is something that she's discussed with you, then I don't think it would be inappropriate at all to tell her that you think she looks great. I'm sure she'll be happy to hear it. If she never mentioned the surgery to you, but you heard through other sources that she had it, maybe a simple "Hey, you look great" will do - rather than a more specific comment.
