I'd assume compound fracture. First things first, make sure the danger has stopped. Check the horse isn't about to dance on the pair of you. Call for help. Then you're going to want to stop the bleeding, and make sure that the casualty isn't going to move, so you need to get them to keep as still as possible to prevent further damage.

First is DON"T move victim, Possibility of neck or back injury. Skull fracture and concussion would be next to consider and then likely a compound fracture of the leg if there is blood present.
