How do you get a US Passport if you live OUTSIDE the US and cannot fly back to get one (because you need one to get in)?

You can go to the US Consulate in the country you are in.

I'd go to the nearest US embassy and talk with them about it.

Go to either an embassy or consulate with as much ID as you can provide.

Contact US Citizen Services at the US Embassy or Consulate in the country you are in. That is what they are there to do. (One thing anyway.)
