im pretty sure you have to go to a booth. although some photograph shops do these aswell now. i hate having my passport photo done they always make me look really white lol

I think to get it correct you need to get it done in a booth, remember the background must be white, and you need to be certain a distance away and head and shoulders only etc, hey you will need to remove your sunnies and hat........ i know you like your hats lol

No, you don't have to go to a booth just have to make sure that it's printed on photo paper and of the correct size. If you notice the shops now days take it on a digital camera, but they just have a nice little photo printer. Any place that requires a "passport photo" should give you the exact requirements in terms of size of the photo, size of the face in the photo, and the type of paper/print required. The end result is all that matters, process is irrelevant.

It depends who it is for. I have experienced the passport office is very strict and may reject it for the slightest deviation from their requirements.

Some nations accept only certain types of photos for passports. Other nations take the digital photo and print it in to the document. Many different systems abound.... what is your citizenship and what passport are you wanting?
