Is it possible to make it so that when you DR someone, or they DR you, a reason has to be given?

No that is not an option here. The main reason we get down-rated is because someone does not agree with what we posted. The down-rating hurts them more than it does the one getting the down-rate because it does not affect your point value but it does their percentage rating. Many misunderstand that the box to the right of the questions and answers as points but they are not points those are ratings. The positive ratings create points but the down-ratings only make the post look bad. Many members see these and read them and if they are not bad will add more positive ratings to bring it back up. which adds even more points that they may not have gotten anyway. Sure it is annoying to get them as no one likes to believe their questions and answers are bad but we all get them because we can not please everyone.
