Should all Americans have free healthcare(employed or not)?

• If health care is free, how do you incentivise healthy lifestyle choices? I am not interested in subsidizing smokers' health care costs through my taxes.

• we already have a program for the poor, it is called Medicaid.
  currently 40 million Americans are on Medicaid. How many poor people do we have ?
  
  Medicare is another government run health program, covers all the old people, even if they are wealthy. 45 million are enrolled today.
