No. Lies are concious, you know you are lieing or distorting the truth when you do so. That is simply human error.

I wouldn't call it a lie, exactly..though, technically I guess it would be, but I'd call it a mistake. Especially if I had every intention of doing it, but it just happened to slip my mind.

Only a bitter, mean person would classify that as a lie. The road to hell is paved with good intentions as they say, but we all have good intentions and then slip up once in awhile. This is in a totally different classificaiton than a lie.

No. A lie is intentional. It becomes a falsehood. If you still plan on doing it, it becomes delayed. If you continue to forget to keep your promises, it becomes a bad reputation and lack of credibility.

No, if there was no mention of when you were going to do it then you can still be going to do it ten years from now so it would not be a lie.
