locutus has the episode right. However, it was not caused by the crew eating bad food. It was caused by Dr. Crusher accidentally creating the condition by treating a genetic disorder that Lt. Barclay had. However, instead of turning on a set of genetic instructions that was supposed to be operational, it turned all of them on (DNA actually has a lot of inactive information, held over from evolution). The condition became known as "Barclay's Protomorphosis Syndrome".
