I need to file my 2004 state taxes and I lost my w-2 form. My previous employer is being non responsive. How do i obtain a copy of my form so that is shows state information?

    The most efficient way to obtain a copy of your W-2 form is to call the IRS and explain that your former company is non responsive ---- they will provide you with a copy of what the employer reported
