I'm not sure about a long term one over a few days but let me tell you, a short term one is painful as hell. I had one done like that as a kid and I was in a lot of pain. I don't remember much of it but it was not pleasant. I will say a 7.5 on the scale. I think that it will be less painful if you have it left in for a few days, as it's specifically made to be lived with. Mine was more an emergency surgical type of situation.

I had a catheter fitted after my back operation and i dont remember it being painful on insertion or removal so will give it a 1....but saying that i have no idea how painful for a man could be on the uncomfortable side.

I had one inserted when I was still under anesthesia so I didn't feel it. It was about a 1 or 2 while it was in. Sure took care of that prostrate problem while it was in. LOL When they took it out (male nurse) it was about a 2 or 3. No big deal!!!

Foley catheters placed intra-operatively are done so once the patient is under general anesthesia, so it is painless. If properly removed (beyond the scope of this answer), there is minimal pain, i.e. 1-2 on the pain scale, during removal. It may be painful to urinate (urodynia) the first few times after a catheter has been removed.
