Get them to a clean source of water to rinse the wound, take some fabric from whatever you can find that's clean and press it to the wound to try and staunch the flow, then find a way to get them to a hospital.

Check the area to make sure it is safe, check to see if they are "ok" (as one can be in that situation). Call for help. If conscience, ask for permission to perform basic first-aid. If not, perform basic first-aid/CPR.

Talk to them and see if they are coherent. If so, my first focus would be to stop the bleeding. If they could walk, we'd walk together to find a phone and call for an ambulance or someone they knew to help get them to the hospital. If they were unconscious and there was no one to go for help, I'd run as quickly as possible to the nearest phone and call 9-1-1, to get a paramedic there.

quite simple....since the gentleman that was bleeding to death is likely going to die in this situation anyway, I would use him as a shield to appease the mastiffs off the start. As they are eating him, I would run and jump, putting 1 foot on the edge of the cave ledge and pushing myself far enough in to lift myself in knowing that mastiffs have no jumping skills to speak of due to their height and I'm a 6'5" marine so I'd have no issues getting into the cave to establish a "base camp" as I plan my attack on the mastiffs at nightfall :)
