It means that you have a leaking wheel seal. You will need to replace the seal. While you're there, do the bearings too, because they'll more than likely go soon anyway especially if they've spent any significant amount of time low on lubrication. From the sounds of things, they have.

You're reading If there is visible grease coming from behind the lugnut protecter (hubcap), on my F150. What does this mean? Do I need to have the wheel bearings re-greased? The truck is rear wheel drive.
